Filler Dissolving

Expert Filler Dissolving to Restore Natural Facial Balance

Hyaluronic acid fillers have become one of the most popular cosmetic treatments available today, helping restore volume, soften facial lines, and enhance natural contours. When performed thoughtfully, filler can produce beautiful, subtle, and long-lasting results. As the use of filler has become increasingly widespread, however, many patients are seeking expert guidance on how to manage filler that no longer reflects their aesthetic goals.

At Treiber Dermatology, filler dissolving is not viewed simply as a corrective procedure—it is often the first step toward restoring natural facial balance. Our practice has become a referral destination for patients who have previously received filler elsewhere and are looking for an experienced cosmetic dermatologist to evaluate whether dissolving older filler may improve their appearance before considering future treatment.

Whether addressing migrated lip filler, persistent under-eye fullness, facial asymmetry, or an overfilled appearance that has developed gradually over time, every treatment plan is individualized. The goal is not simply to remove filler, but to help patients achieve refreshed, natural-looking results that complement their unique facial anatomy.

Hyaluronidase is an enzyme that safely breaks down hyaluronic acid, the ingredient found in most modern dermal fillers, including products within the Juvéderm® and Restylane® families.

Once injected into the targeted area, hyaluronidase begins dissolving the filler, allowing the body to gradually absorb it over the following days. The amount of enzyme used depends on the type of filler, the amount present, the location being treated, and the desired degree of correction.

Because hyaluronidase works only on hyaluronic acid fillers, it is not effective for biostimulatory fillers such as Sculptra®, Radiesse®, or other non-hyaluronic acid products. Understanding exactly which product was previously injected is helpful whenever possible, although treatment can often still be performed even when records are unavailable.

Patients choose to dissolve filler for many different reasons, and not all involve complications.

Increasingly, patients come to our practice because they are no longer happy with filler that was placed months or even years earlier. Some feel they have gradually developed an overfilled appearance. Others notice persistent puffiness, migrated filler, or facial contours that no longer appear balanced. Many simply want to return to a more natural look before deciding whether additional filler is appropriate.

Common reasons patients seek filler dissolving include:

  • Filler migration, particularly in the lips and under-eye area
  • An overfilled or “puffy” appearance
  • Asymmetry
  • Lumps or contour irregularities
  • Persistent fullness from filler placed years earlier
  • Desire to return to a more natural appearance
  • Preparation for cosmetic procedures such as eyelid surgery
  • Creating a fresh foundation before carefully replacing filler
  • Rare vascular or medical complications requiring urgent treatment

For many patients, dissolving filler is not the end of treatment—it is the beginning of a more thoughtful, individualized approach to facial rejuvenation.

As cosmetic dermatology has evolved, so has our understanding of how dermal fillers behave within the face.

Hyaluronic acid fillers were once believed to dissolve completely within several months to a year. We now recognize that, in some patients, filler may persist significantly longer than originally expected. Rather than disappearing entirely, it may gradually soften, shift, or accumulate over time, contributing to changes in facial contours that were not immediately apparent after treatment.

Patients are often surprised to learn that filler injected years earlier may still be influencing their appearance today. This is particularly common in areas such as the lips, tear troughs, cheeks, and jawline, where repeated treatments over time can create more volume than intended.

At the same time, aesthetic preferences have shifted. Many patients who once desired a fuller appearance now seek a more subtle, refreshed, and natural look.

As a result, Treiber Dermatology increasingly evaluates patients who have received filler elsewhere and are looking for an experienced physician to determine whether dissolving older filler is the best first step. In many cases, allowing the face to return closer to its natural foundation before reinjecting produces softer, more balanced, and more predictable results than continuing to layer additional filler over existing product.

One of the most important decisions is determining whether filler should be dissolved completely—or only partially.

Many patients assume that every treatment requires complete removal, but that is often unnecessary. Sometimes only a small amount of hyaluronidase is needed to soften an edge, improve symmetry, reduce migration, or restore more natural contours while preserving the areas that still look beautiful.

Every face is different, and every treatment plan is customized.

Patients frequently seek consultation at Treiber Dermatology specifically because filler dissolving requires both technical expertise and aesthetic judgment. Our goal is never simply to remove volume. Instead, we carefully evaluate facial anatomy, previous filler placement, and long-term aesthetic goals to determine the approach most likely to achieve a natural result.

When additional filler is desired after dissolving, it is often strategically replaced only where it is truly needed, creating a more harmonious and refined outcome.

Filler dissolving is also commonly recommended before certain facial procedures, particularly blepharoplasty (eyelid surgery).

Excess filler around the eyes or midface can sometimes obscure the natural anatomy that surgeons rely upon when planning surgery. Dissolving unwanted filler beforehand allows the underlying facial contours to be evaluated more accurately and may contribute to a more predictable surgical result.

If additional volume is desired after healing, filler can always be thoughtfully reintroduced at a later time.

Filler dissolving is performed comfortably in the office and typically requires only a few minutes.

Following consultation, hyaluronidase is injected into the targeted areas using a cannula. Mild swelling, stinging, tenderness, or bruising is common immediately after treatment and generally resolves within several days.

Most patients begin noticing improvement within 24 to 48 hours, although complete results may continue to develop over the following week. Depending on the amount and type of filler present, more than one treatment session may occasionally be recommended to achieve the desired result.

A Thoughtful Approach to Natural Results

Cosmetic dermatology is not simply about adding volume—it is about creating facial harmony that looks refreshed, balanced, and authentically you.

At Treiber Dermatology, filler dissolving is viewed as an important part of comprehensive facial rejuvenation rather than simply a corrective procedure. Whether helping a patient dissolve years of accumulated filler, preparing for eyelid surgery, or creating a fresh foundation before reinjection, every recommendation is guided by careful evaluation, technical precision, and aesthetic judgment.

Sometimes achieving the best result means adding volume. Other times, it means taking volume away. Knowing when to do each—and having the experience to do it thoughtfully—is what allows for consistently natural, elegant results.
