If there is any concern that a mole could represent melanoma or another type of skin cancer, cosmetic removal is not appropriate. Instead, a biopsy or complete surgical excision may be recommended to establish an accurate diagnosis and ensure appropriate treatment.

Many patients are surprised to learn that the reason for removing a mole often determines whether insurance provides coverage.

When a mole is suspicious for skin cancer, repeatedly irritated, bleeding, painful, or otherwise medically necessary to remove, insurance may provide coverage depending on the individual’s plan.

When the primary reason for removal is cosmetic improvement, however, the procedure is generally considered elective and is not covered by insurance.

Shave Removal

Many raised benign moles can be removed with a shave excision, a technique in which the mole is carefully shaved flush with the surrounding skin after local anesthesia.

Because shave removal usually does not require stitches, healing is often straightforward and recovery is relatively quick. This technique can produce excellent cosmetic results for carefully selected raised moles.

Since the deeper portion of the mole may remain beneath the skin, there is a small possibility that pigment or elevation can recur over time. For many patients, however, the cosmetic result makes shave removal an excellent option.

Surgical Excision

For larger, deeper, or certain cosmetically challenging moles, complete surgical excision may provide the best long-term result.

This technique removes the entire mole through a carefully planned elliptical incision, followed by meticulous layered closure with sutures.

Although excision results in a fine linear scar, it also provides complete removal of the lesion and minimizes the likelihood of recurrence. In many situations, a well-healed linear scar is ultimately less noticeable than a recurrent or partially flattened mole.

Choosing between shave removal and surgical excision requires both surgical judgment and aesthetic experience.

Any procedure that removes a mole creates some degree of scarring because the skin must heal after tissue is removed. The goal is not to eliminate scarring entirely, but to create a scar that heals as inconspicuously as possible.

Even when a mole appears completely benign, it is often submitted to a pathology laboratory for microscopic examination to confirm the diagnosis.
