What Is Melasma?

Melasma is a common form of hyperpigmentation that appears as brown or gray-brown patches on the skin. Although it can occur anywhere on the body, it most often affects sun-exposed areas of the face such as the cheeks, forehead, and upper lip—and occasionally the forearms. It is often triggered or worsened by hormonal changes (such as pregnancy or birth control), sun exposure, and even heat. Melasma is complex and can be challenging to treat. Each case is unique, and successful management requires a thoughtful, individualized approach.

How We Approach Melasma

Because melasma develops from multiple factors, effective treatment requires a comprehensive, layered and personalized strategy. Our physicians carefully evaluate each patient’s skin, triggers, and lifestyle to create a customized plan designed to reduce pigmentation and prevent recurrence.

Our approach often includes:

  • Rigorous sun protection: A UPF-rated, wide-brimmed hat whenever outdoors and consistent use of daily sunscreen.
  • Sunscreen layering: Apply a non-tinted, broad-spectrum sunscreen first (such as a high-quality European chemical formula or a mineral option like ISDIN Eryfotona Ageless), followed by a tinted sunscreen containing iron oxide—such as Alastin Hydratint—to protect against visible and ambient light.
  • Medical-grade brightening serums to target uneven pigmentation.
  • Prescription lightening creams customized to your skin’s needs.
  • Oral tranexamic acid, when appropriate, to help control pigment formation from within.
  • In-office treatments, including gentle chemical peels and laser therapies such as Clear + Brilliant® Permea
  • Daily antioxidant support, such as Heliocare, especially during the summer or before travel to sunny climates.
